Anyone recommend a custom plexiglass vendor for a 4600 transom door

Mine is cracked all all sides... I'm thinking of having a 1" custom plexiglass sheet made with a Maxum logo etched on it. I know I've seen boats with this done.

Anyone know of a good place to order a custom fabricated door? I've Googled it and found plenty of matches, but would rather get a referral if someone has done it.
